Services

• Healthcare Management and maintenance appointments: for your chronic health conditions typically require 15-30 minutes  

• Motor vehicle accidents: appointments usually last 30 minutes and require the name of your auto insurance company, claim address, adjustors name/ phone number when scheduling.

• Pain diagnostics: appointment times vary.

• Physical exams/ screenings: 30 minute appointment, may include papsmere; includes yearly risk factor assessments based on your personal and family histories and a variety of screenings per the recommendations for hypertension, diabetes, complications associated with obesity, heart disease, female and male cancers, sexually transmitted diseases, as well as risk factor management counseling. Please check with your insurance prior to scheduling to ensure your eligibility for coverage

• Referrals and refills: (require an appointment and are not routinely approved by phone): 15 minute appointment; please schedule prior to your medication expiration to ensure no gaps in your treatment plan.

• Same day sick visits: for your convenience: 15 minute appointment, M-W, F, and S; available for problems such as fevers, uncomplicated infections, acute respiratory infections, abdominal and pelvic pain, uncontrolled diabetes/ hypertension, colds, flus. Call by 11:00 to ensure availability.

• TB Screening/ Immunizations: for flu and other communicable diseases: 15 minute appointment

• Weight loss Consultation/ Programs: initial private consultation: 45 minute appointment, includes screening for behavioral eating disorders, complications of obesity and co-morbid conditions such as sleep apnea, diabetes, heart disease, back and neck pain, and circulation disorders. Individualized programs may include medications for weight loss, diet management, prescriptions for exercise, screening and preparation for bariatric surgeries.  Followup visits are 30 minutes and are scheduled as needed

• Work Injury: 30 minute appointment to assess your symptoms; also requires authorization from your employer. Please provide your insurance carrier, policy number, and adjustors name/ phone number when scheduling.

All services can be discounted for those underinsured or without medical insurance so that you can still access the care and screenings that you need to maintain healthy bodies and wellbeing.

To request medical records, please call our office for a medical release form. An associate will contact you regarding any copy fees. Forms such as those required by your employer, insurance applications, school forms, etc. may be faxed or personally submitted and also require a medical release.

Please allow 10 business days for completion.   

If you require a return to work/ school note, you must be seen within 48 hours of your absence
